Your Future Health Team


Future Health is built on the belief that meaningful, long-term health change rarely comes from one appointment alone.

Following your assessment, Dr Kate may recommend additional support from trusted members of the Future Health collaborating team to help translate your personalised strategy into lasting results.

Every recommendation is tailored to your individual goals, health priorities and stage of life. For some people, regular medical reviews provide all the support they need. Others may benefit from one additional program, while some may choose a more comprehensive multidisciplinary approach.

Ongoing support can help accelerate progress and turn recommendations into lasting habits.

Physical Capability Program


PMQ Health programs have been developed to provide structured, evidence-based pathways.

Each program combines comprehensive assessment, personalised exercise prescription, physiotherapy-led treatment, objective outcome measures and regular reassessment to help patients achieve meaningful, long-term outcomes.

Every participant follows the same clinical framework:

Assessment → Exercise Prescription → Supervised Training → Progress Review → Ongoing Progression.

Your Future Health Assessment already provides a comprehensive evaluation of your strength, fitness, balance, mobility and overall physical capability, along with an individualised exercise prescription tailored to your goals and health needs.

Because this assessment has already been completed as part of your personalised Future Health Strategy, you can move straight into the PMQ Health exercise programs without needing a separate initial assessment. These initial assessments will be used to assess and compare your ongoing progress by the PMQ Health team.

All programs are eligible for Private Health fund rebates.



Nutrition Optimisation Program


Nutrition recommendations are individualised according to your body composition, metabolic health, lifestyle, blood results and personal goals.

Rather than providing generic advice, the focus is on creating realistic eating patterns that support long-term health.

Your program includes:

  • Comprehensive ongoing nutrition review

  • Updated personalised meal plan every 3 months

  • 30-minute telehealth review consultation to discuss nutrition changes

  • Ongoing adjustment as your goals evolve

Please note, this is a repeat review after the initial review included in Future Health strategy.

Behaviour Change Program


Knowledge doesn't always translate into action. Behaviour change coaching helps bridge the gap between knowing what to do and consistently doing it.

Sessions are practical, collaborative and focused on building sustainable habits that fit your life.

Your program includes:

  • One 45-minute initial consultation

  • One 30-minute follow-up session approximately two weeks later

  • One 45-minute review session 4–6 weeks after your follow-up

Support may include:

  • Habit formation

  • Motivation

  • Accountability

  • Emotional eating

  • Stress management

  • Goal setting

  • Overcoming barriers to change

  • Building long-term confidence and self-efficacy

All sessions are eligible for Private Health fund rebates.


Women’s Exercise Program


These programs are designed to improve the physical capabilities that matter most for healthy ageing, including muscle mass, strength, cardiovascular fitness, balance, mobility and overall physical function. They are designed for those who would prefer to do their training in their own gym or at home.

10-Week Individualised Everfit Program — Intro Offer $200.

Includes:

  • Individualised Everfit program based on Future Health assessment results 

  • Program delivered through the Everfit app 

  • Home or gym-based training options 

  • Program updated every 5 weeks to allow for optimal progression

  • Fortnightly app-based check-ins for accountability, progression and problem-solving
